Conquering Falls Through Effective Care Planning

Target Audience: Administrator, Admissions, Nursing, Therapy, Restorative Staff, QA/Risk Management Staff, Social Services and Staff Development

This 8-week series, running February-March 2022, guides the interdisciplinary team through the elements of an effective falls prevention program. Sessions focus on assessment strategies that guide targeted risk-based interventions, responding to a fall event, and promoting a fall prevention environment. Presented by a team of nurse and therapist experts, this series aims to provide the insights and tools needed to implement improved falls prevention programs in the SNF/LTC/AL setting.

Sessions will address integrating nursing and therapy assessments to determine individual fall risk factors, addressing risks using evidence-based practice, person centered care planning and applying targeted interventions to address risk factors such as dementia, low vision, balance impairment, incontinence as well as improving safety through interventions that consider resident preferences, and application of root-cause analysis when falls occur.

February 23rd Session
This session focuses on effective care planning for fall prevention using an interdisciplinary approach with an emphasis on addressing resident goals, preferences and fall risk factors. Regulatory requirements and common areas of deficiencies will be reviewed related to falls care planning with suggested strategies to promote sustained compliance. Discussion will address complicating factors such as restraints, cognitive impairment, supervision and repeated falls in addition to considerations for ensuring interventions are reasonable, effective and consistently applied. Best practices for maintaining and updating care plans using an interdisciplinary approach, and in tracking/documenting outcomes of interventions will be covered.

Projected Learning Outcomes/Course Objectives: At the conclusion of this session, the learner will:

1. Identify regulatory requirements related to Care Planning as applied to fall prevention
2. Develop an effective person-centered plan of care that focuses on the fall prevention goals and addresses the specific fall risks of each resident
3. Implement effective systems for IDT involvement in fall prevention care plan development and revision
