Winning the Fall Prevention Battle
Every Wednesday beginning February 2nd

Target Audience: Administrator, Admissions, Nursing, Therapy, Restorative Staff, QA/Risk Management Staff, Social Services and Staff Development

This 8-week series, running February-March 2022, guides the interdisciplinary team through the elements of an effective falls prevention program. Sessions focus on assessment strategies that guide targeted risk-based interventions, responding to a fall event, and promoting a fall prevention environment. Presented by a team of nurse and therapist experts, this series aims to provide the insights and tools needed to implement improved falls prevention programs in the SNF/LTC/AL setting.

Sessions will address integrating nursing and therapy assessments to determine individual fall risk factors, addressing risks using evidence-based practice, person centered care planning and applying targeted interventions to address risk factors such as dementia, low vision, balance impairment, incontinence as well as improving safety through interventions that consider resident preferences, and application of root-cause analysis when falls occur.

The Series

  • February 2 - Assessing Fall Risk Using Evidence Based Practice

This session will review best practices for fall risk assessment with an emphasis on identifying those risk factors that may be proactively addressed with appropriate, preventative interventions. Key elements of the nursing fall risk assessment, medication review process, as well as therapy’s role in testing and providing objective measures of potential impairments in areas such as mobility, balance, cognition, vision, sensory perception, and fear of falls will be reviewed. Recommended resources for evidence- based testing protocols will be discussed with an introduction to applications of the fall risk assessment into actionable care plan strategies for reduced incidents and improved safety.

Projected Learning Outcomes/Course Objectives: At the conclusion of this session, the learner will:

1. Identify key risk factors for geriatric falls and assessment processes to objectively measure individual risk.

2. Understand methods for applying evidence-based practice to falls prevention and management efforts using an interdisciplinary team approach.

  • March 23 - Tactical Plan for Sustained Falls Prevention Through QAPI
