Target Audience: Administrator, Admissions, Nursing, Therapy, Restorative Staff, QA/Risk Management Staff, Social Services and Staff Development

This 8-week series, running February-March 2022, guides the interdisciplinary team through the elements of an effective falls prevention program. Sessions focus on assessment strategies that guide targeted risk-based interventions, responding to a fall event, and promoting a fall prevention environment. Presented by a team of nurse and therapist experts, this series aims to provide the insights and tools needed to implement improved falls prevention programs in the SNF/LTC/AL setting.

Sessions will address integrating nursing and therapy assessments to determine individual fall risk factors, addressing risks using evidence-based practice, person centered care planning and applying targeted interventions to address risk factors such as dementia, low vision, balance impairment, incontinence as well as improving safety through interventions that consider resident preferences, and application of root-cause analysis when falls occur.

March 9th Session (Part 2) 
This session focuses on applying effective, person-centered fall prevention interventions based on resident specific risk factors and fall history. In this second of a 2-part discussion of intervention planning, cognitive, psychosocial and environmental risk factors and interventions to address them will be reviewed including dementia, depression, fear of falls, and promoting optimal sleep health. Best practices for applying information gathered from evidence-based assessments, trends of past falls and connecting the dots between clinical findings and fall prevention approaches will be emphasized.

Projected Learning Outcomes/Course Objectives: At the conclusion of this session, the learner will:

1. Identify key considerations for fall prevention related to dementia and applying effective interventions based on remaining cognitive abilities.

2. Understand targeted interventions for common cognitive and psychosocial fall risk factors.

3. Determine appropriate interventions based on clinical assessment data, falls history trending and person-centered preferences and routines.
