Care Coordination between licensed care settings is a regulatory requirement yet an ongoing challenge for Home Health, Hospice, Assisted Living Facilities (ALFs) and Skilled Nursing Facilities (SNFs).

The CT Association for Healthcare at Home is collaborating with the CT Assisted Living Association and LeadingAge Connecticut to offer two half days sessions to review the Care Coordination regulations as well as engage the attendees in discussion of strategies to optimize care coordination when home health or hospice provides services in an ALF or SNF.

  • The morning session will feature legal expert Heather Berchem (Murtha Cullina LLP) who will provide the foundation of the regulatory requirements (both federal and state) for care provision in an ALF by Home Health or Hospice. This will be followed by panel discussions to share best practices and address challenging scenarios.
  • The afternoon session will focus on Hospice Care within the SNF setting. Legal expert Maureen Weaver (Wiggin and Dana) will first review the regulatory requirements (both federal and state) and a panel discussion will follow to once again engage the participants in sharing best practices and potential strategies to address care coordination issues.

Objectives:

  • Understand the federal and state regulatory requirements for Assisted Living Facilities, Home Health Care and Hospice Agencies
  • Utilizing panel provider discussions, learn ways to optimize care coordination across provider types.
  • Address complicated care scenarios using examples offered by the participants.

Heather Berchem, Partner at Murtha Cullina LLP
As Chair of Murtha Cullina's Long-Term Care Practice Group and a member of the Health Care and Information Security and Privacy Practice Groups, Heather Berchem advises nursing homes, assisted living communities, CCRCs, hospitals, and home health care and hospice agencies on a wide range of regulatory, compliance, risk management and reimbursement issues.

Maureen Weaver, Partner at Wiggin & Dana
Maureen Weaver is recognized as a leading health care lawyer at both the state and national levels. She chairs Wiggin and Dana's Health Care Practice Group and advises health care providers on complex regulatory, patient care, reimbursement, compliance, operational, and corporate matters. Maureen is particularly experienced in representing long-term care providers, including continuing care retirement communities, nursing homes, assisted living facilities, and senior housing developers.
